Constitutional first cause
The Engine begins when: Medication for opioid use disorder is initiated or substantially changed, creating obligations that extend beyond the initiating encounter. Not at relapse. Not at overdose. At initiation.
Current systems can demonstrate: buprenorphine prescribed; discharge completed; referral written. They often cannot demonstrate: who accepted ongoing prescribing responsibility; whether medication access actually existed; whether the first outpatient encounter occurred; whether the accountability chain remained intact after discharge. The failure is not starting treatment. It is failing to demonstrate continuity of the obligations created by treatment initiation.
