Constitutional first cause
The Engine begins when: A patient reaches a transition point requiring transfer of longitudinal specialty responsibility from pediatric to adult care. Not when the patient misses an appointment. Not when complications occur. When the transition becomes constitutionally necessary.
Current systems can demonstrate: referral sent; records transferred; appointment scheduled. They often cannot demonstrate: whether the adult team accepted ongoing responsibility; whether pediatric accountability closed appropriately; whether the patient was actually established in the new system; whether longitudinal care remained uninterrupted. The accountability unit is not the referral. It is the transfer of continuing responsibility.
