Constitutional first cause
The Engine begins when: A pathogenic or likely pathogenic germline variant is confirmed and disclosed, creating new obligations for the patient, clinicians, and potentially biologically related family members. Not when cancer develops. The obligation begins when the verified genetic finding enters the system.
Current systems can demonstrate: the genetic test was performed; the report exists; counseling occurred. They often cannot demonstrate: that long-term risk-management obligations were accepted; that surveillance remained active; that preventive options were revisited as circumstances changed; that cascade-testing obligations were addressed. Again... The constitutional unit isn't the gene. It's the obligation network created by verified knowledge.
