The Engine begins when: A patient develops a condition in an austere environment requiring ongoing reassessment of whether field management remains appropriate or evacuation has become necessary. Not when the helicopter launches. Not when rescue arrives. When the evolving condition creates competing operational obligations.
Current systems can demonstrate: patient assessed; evacuation requested; transport completed. They often cannot demonstrate: why continued field care remained justified until a particular moment; when changing evidence altered the obligation; who accepted the evolving operational decision; whether the transition occurred at the appropriate constitutional threshold.
